School Report Forms :
Most, though not
all, colleges and universities include a Secondary School Report as
part of the application. Occasionally, these forms are labeled "Transcript
Request Form".
Typically,
these forms ask counselors to note certain information about the student
and may include check off lists to rate the student in several areas.
**Almost
every Secondary School Report Form has one part that must be completed,
and often signed, by the student. This section is usually the first
part of the form and requests basic information like name, address,
SSN, etc. Please fill out these sections before giving the forms to
your counselor.
